SEO for therapists, minus the mythology

Three channels bring therapy clients from search in 2026: Google's local pack, classic organic results, and — newly and fast-growing — AI answers. Each responds to different work. This guide is the distilled version of the research we built our own SEO engine on.

Channel 1: the local pack (biggest, least about your website)

When someone searches "therapist near me," the map results are decided mostly by proximity and your Google Business Profile— GBP signals carry roughly a third of local-pack weight in practitioner surveys, more than any on-site factor. The work: claim the profile, choose "Psychotherapist"/"Counselor" categories, keep your name, address, and phone character-identical to your website, post monthly, and ask satisfied former clients or colleagues for reviews where your board allows it. Nothing on your website substitutes for this.

Channel 2: organic results (where your site does the lifting)

The winning on-site pattern is unglamorous: one substantial page per specialty — "anxiety therapy in Portland" — with genuinely distinct content per page: the symptoms that bring people in, your modalities, sourced statistics, and a real FAQ. Thin pages that differ only by a swapped keyword get classed as doorway pages and filtered. Under the hood, specific schema types (MedicalBusiness rather than genericLocalBusiness, Service, FAQPage) are weighted higher by both Google and AI engines.

Channel 3: AI answers (new, compounding)

AI Overviews now appear in over half of US searches, and studies put AI-referred visitor conversion at ~4× organic. AI engines don't rank pages — they cite them, and they cite predictably: answer-first sections (the first two sentences carry the weight), FAQ markup (cited at roughly 41% vs 15% without, in 2026 measurements), statistics with named sources, and — crucially — server-rendered HTML, because most AI crawlers don't execute JavaScript. If your site builder renders content client-side, AI engines may see an empty page. Freshness matters too: most AI citations go to pages updated within the year.

The complete work list

  1. Google Business Profile: claim, categories, NAP consistency, monthly posts, reviews.
  2. One substantive specialty page per focus area, answer-first, with FAQs.
  3. Unique title/description per page; your city in the home title if you practice locally.
  4. MedicalBusiness + FAQPage schema, server-rendered.
  5. A blog that answers one real client question per post — freshness feeds AI citations.
  6. Sitemap in Search Console; instant-indexing pings (IndexNow) on publish if your platform supports them.
Items 2–6 are automatic on PracticeWoven: specialty pages generate with distinct long-form content per focus area, every page ships MedicalBusiness/FAQPage schema in server-rendered HTML, and publishing pings IndexNow. Item 1 is yours — no website can do GBP for you, and anyone who claims otherwise is selling something.

Common questions

What matters most for a therapist's SEO?

Your Google Business Profile. Local-ranking surveys consistently weight GBP signals around a third of the local pack — more than anything on your website. Claim it, pick the right categories, keep your name/address/phone identical to your site, and gather reviews.

Do AI chatbots really send therapy clients now?

Increasingly. AI Overviews appear in over half of US searches, and AI-search visitors convert at roughly 4× organic visitors in published studies. AI engines cite sources rather than rank them — pages with answer-first structure and FAQ markup get cited several times more often.

Should I write a page for every specialty?

Yes, if each page is substantive — different symptoms, modalities, and FAQs, not a city-name swap. Google's doorway-page guidance punishes thin variants; long-form pages that genuinely differ are the single most effective on-site pattern for 'specialty + city' searches.

How long until SEO works?

Local results can move within weeks of a GBP cleanup; organic rankings for specialty pages typically take months. The honest schedule: do the one-time on-site work now, then put recurring energy into GBP posts and reviews.
